Job Title: Nurse Practitioner (NP) – Primary Care for Maricopa County, AZ | Seeking providers living in Chandler, Mesa, Tempe, or Gilbert, AZ

Full-time hours | Temp to Perm | Monday–Friday | 9 AM – 5 PM | In-home & community-based visits | requires that providers be willing to drive/cover up to50-minute radiusfrom their home

Overview

We are seeking an experienced Nurse Practitioner (NP) to provide comprehensive primary care services to patients in home-based and community settings. The ideal candidate will play a crucial role in delivering high-quality, patient-centered care, promoting continuity of care, and improving overall patient outcomes.

Key Responsibilities

  • Conduct in-home and community-based visits (minimum of 25 per week) in assisted living facilities, skilled nursing facilities, group homes, and delegated locations.
  • Perform comprehensive patient assessments, including medical history reviews, physical examinations, and risk evaluations.
  • Diagnose and treat acute and chronic health conditions while developing personalized, evidence-based care plans.
  • Manage chronic diseases such as diabetes, hypertension, and cardiovascular disorders, ensuring optimal patient education and disease control.
  • Prescribe and adjust medications while providing guidance on administration and potential side effects.
  • Promote preventive healthcare, including immunizations, screenings, and lifestyle modifications.
  • Deliver patient education on disease management, healthy habits, and self-care strategies.
  • Serve as the primary care provider of record, ensuring seamless coordination of care.
  • Maintain accurate, up-to-date electronic medical records (EMR) and complete documentation within 48 hours of each visit.
  • Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families.
  • Participate in quality assurance initiatives and adhere to healthcare regulations and standards.
  • Provide after-hours, on-call support when needed for urgent care needs or initial assessments.
  • Ensure reliable transportation to patient locations and adhere to scheduled appointment times.
  • Strictly follow all company policies, procedures, ethical standards, and compliance regulations.

Qualifications

  • Active, unrestricted NP license in Arizona, free of clinical or board restrictions.
  • Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
  • Ability to obtain autonomous provider status within six months of eligibility.
  • Minimum two years of primary care or longitudinal care experience preferred.
  • Active DEA license and Basic Life Support (BLS) certification required.
  • Strong clinical assessment, diagnostic, and patient education skills.
  • Proficiency in electronic medical records (EMR) systems.
  • Valid driver’s license with a clean driving record and access to reliable transportation.
  • Ability to pass background checks and drug screening per employer and regulatory requirements.

Preferred Qualifications

  • Experience with HEDIS/STAR measures, home-based visits, and Hierarchical Condition Categories (HCC) documentation.

Physical & Work Environment Requirements

  • Ability to lift and carry up to 20 lbs and perform duties that involve standing, bending, kneeling, and driving.
  • Work is performed in patients’ homes, with potential exposure to environmental challenges.
